Based on regulations, you may be required to enter additional information to complete this page.
Regulations require specific details for a hazardous material shipping description. It is your responsibility to review and verify that this information is accurate and complete according to applicable regulations.

Use the drop-down menu if the material has a Subsidiary Hazard Class.
If more than 1 packing group is
applicable, use the drop-down menu to select the appropriate number.
If the Hazardous Substance Name field is not already filled in, and your commodity needs to be flagged as a reportable quantity, click ‘Yes’ and add the Hazardous Substance Name.
Limited quantity is for intermodal shipments with small packages that
are below the limits that require
labeling.
Indicate if the material is a Poison Inhalation Hazard and select the Zone if it is.
If your commodity is a Marine Pollutant, select ‘yes’ and type
the name of it in the field provided.
Net Explosive Quantity applies to Class 1 commodities in Canada.
Enter the Weight/Volume of a package and use the drop-down box to select the unit of measurement.

Use the radio buttons to tell us if you are shipping an empty car or package that contains residue from a hazardous load, as it is considered hazardous until it has been cleaned.
Enter the container initial & number. If shipping a flat car with more than 1 container on it, the container is the package.
Compartment location applies to tank cars with multiple compartments.
Enter Damaged Initial & Number only if shipping a damaged rail car
containing residue that is not moving on its own wheels.

If your shipment has an origin or destination of Canada, the Emergency
Response Plan (ERP) Number and phone number may be required for loaded
or empty shipments, based on the commodity. Refer to Canadian hazardous
commodity regulations.
